Our Cancellation Policy:

  • 24-Hour Notice Required: Please provide at least 24 hours' notice for appointment cancellations

  • Late Cancellation Fee: Full session fee may apply for cancellations with less than 24 hours' notice

  • No-Show Fee: Full session fee applies for missed appointments without prior notice

  • Emergency Exceptions: We understand that true emergencies occur and will work with you on a case-by-case basis

7. Intellectual Property

11. Insurance & Payment Terms

Insurance

  • We are in-network providers with Aetna and Optum/United Healthcare, along with various carveout plans that contract with Aetna and Optum/United Healthcare

  • Insurance benefits are verified through our Simplelntake system or the insurance company's designated site.

  • You are responsible for understanding your benefits and any applicable deductibles or co-pays

  • If a coinsurance or other insurer is not disclosed, resulting in the denial of a claim for rendered services, you are responsible for our out-of-network fee, as outlined in your informed consent document.

Payment

  • Payment is due at the time of service unless other arrangements have been made

  • We accept credit cards and HSAs. We maintain a valid card on file to charge at the time of the session.

  • Outstanding balances over 30 days may be referred to a collection agency.

  • If your card on file denies the transaction for any rendered services, Duality reserves the right to pause services until the debt is paid in full and a new payment method is established.

12. Termination

We may suspend or terminate your access to our website or services if you violate these terms or our clinical policies. You may discontinue services at any time, though we encourage discussing termination in session when clinically appropriate.
